Hi, from your description she definitely needs Psychiatric evaluation. This may be Dysthymia as opposed to Depression. Having said that however she will need personal evaluation by a Psychiatrist for a proper diagnosis.
She may require medication for a short period, however she will benefit greatly from counseling or therapy especially given the fact that she had a rough childhood.
Some of her patterns of behaviour like her fear of failure and mood fluctuations can easily be addressed by the therapy.
She has to focus on her strengths. She has been a responsible girl handling so much. It just takes simple steps to understand that she need not give way to the stress she is experiencing.
