As per your history... episodes of depression come first followed by suspiciousness etc. So certainly there is no schizophrenia.  He may be having recurrent depressive disorder with psychotic symptoms. He should be put on antidepressants and antipsychotic both. Can change to another agent from olanzapine.

First, I want to tell you that its not depression but schizophrenia. It is a life long illness and he will need treatment all the time. If medicines are not working, then either we have to increase the dose or change the medicine

Your history is suggestive of a psychotic illness. Medically speaking it can be: - schizophrenia - schizo affective disorder - depression with psychotic symptoms One thing I would like to know is that whether these symptoms have persisted from 11 years or there are periods of complete normalcy in between. As per the treatment if olanzapine alone is not helping we may need to - add another drug or - change it or - increase its dose. Discuss these issues with your local psychiatrist. Maintain regular follow ups.
