This may not be depression, I would suggest you start meditating twice a day for atleast 15mins per session, this will help to focus, start exercising for atleast 30mins and get a hobby, your other symptoms would then be taken care of. Schedule time for studies and a time for play and make sure you follow the it religiously. Make friends if you can. All the best.

Dear Ayushi, This can be an unpleasant time for you. But as with all phases in life, this too will pass. So, dont worry about it. Worrying magnifies the issue. You have said you are unable to concentrate 'comparatively' nowadays. Try to find out what are the significant things which have happened between the time you felt well and now. If there is any problem you are currently facing, try to get help from an elder whom you trust or any counselor. Also mood swings are quite common in teens.
Next Steps
1. Give positive inputs to your mind everyday, by either reading a page from a good self help book or watching 3 motivational videos 2. Do any physical activity which you love for 40 mins everyday. It can be Jogging, cycling, swimming etc. This will release lot of feel good chemicals like endorphins in your brain and you will start feeling energetic 3. You can do some breathing exercises like Brahmari pranayam in the morning and night. It will take hardly 5 mins of your time. But it will help you feel more alert, focused and productive. Above all, take good care of yourself. Be very loving to yourself and never whack yourself or become self critical. Take care Ayushi
