Decompensated chronic liver disease

A pt diagnosed with the above disease is currently admitted to icu and is undergoing failure. He has hypoalbuminia asicitis, pedal oedma slurred speech bilirubin-21 k/c/o- HTN. No medications like albumin, lasik, tazact and much more are not working. The doctors are suggesting liver transplant. But the family is denying. All the reports which are done are raised. And doctor is saying that the pt is undergoing HEPATORENAL SYNDROME. And when the doses are tapered there is no improvement in condition Kindly suggest How can this pt be managed

This patients not only has decompensated cirrhosis but a clinical syndrome called as Acute on Chronic Liver Failure which has hepatic and extra- hepatic (like kidney in this case) organ failures. These patients need early Liver Transplantation without which the survival (in weeks) is very poor. Liver transplantation can only be considered as long as kidneys have mild dysfunction. In cases with advanced kidney failure requiring Hemodialysis, outcomes of liver transplantation alone are dismal.
