Cuts on Penis foreskin

My foreskin on my penis is very tight and keeps getting cuts and inflamation is caused when trying to clean. Redness of foreskin is also there. I am using candid-B cream for itching but these cuts keep coming back and it's very discomforting. Need specialist advise.
