We have to still look at multidrug resistant TB as there may be in vivo in vitro discrepancies or a hyper immune response to tb which is seen in some patients after starting the TN treatment. He needs to be constantly under supervision of an experienced pulmonologist. I will suggest to go to a government tertiary care setting where proper investigations and treatment facilities are available

Nothing to worry. Fever will come down after few days. The clinical improvement like wt gain, etc, shows that, the drugs are acting.pl continue the Att as prescribed by specialist. Fever can be due to toxins released by the bacteria, when they are killed. Pl consult pulmonologist for further evaluation and treatment.

In tuberculosis fever can persist for more then 3 weeks even after ATT. It's normal (evening temperature rise)no need to worry about it. Consult with detailed history
