Dialysis is not decided by
creatinine value alone. It is started only when certain danger signs are present, such as fluid overload, severe breathlessness, very low urine output, high potassium, or acidosis. From what you described, this could be
kidney dysfunction worsened by infection and underlying heart disease. In some cases, it may also be due to a new onset glomerulonephritis.
Next Steps
I strongly advise you to consult a nephrologist immediately or get him hospitalized. The treating nephrologist will evaluate and decide whether dialysis is needed based on his overall condition.
