Talking about suicide need not be labelled as psychiatric issue completely.  There could be other reasons too. First both of you give for a family counselling and after couple of sessions, if it is really an issue with your spouse's mental health the Counsellor might refer to a Psychiatrist. You need to work on convincing your spouse, there is no way of treating her without her presence.
