Chronic kidney disorder

My uncle is 50+ and he has diabestes for over 20 years, he had suffered a stroke in march which led to the detection of Chronic kidney disorder ( CKD). After following medicines and dietary changes even his creatinine level has risen to 11.36. I want to know whether at this stage is it even possible to get a kidney transplant done? if not then what is a better option in this case? urgent!
