Your history is suggestive of bronchial asthma. Uncontrolled bronchial asthma for a long duration can again lead to high blood pressure and diabetes. As you are experiencing increased breathlessness, chest tightness and fatigue it is probably due to exacerbation of your asthma. In such condition just inhaler therapy wont be very effective. You need to consult a pulmonologist at the earliest and get the exacerbation settled with short term nebulizations and systemic steroid therapy.
