Blood clot in foot fingers and swelling

Blood clotting and swelling in the toes, it is very painful and itchy and it is more when the feet are warm and when the feet are cold then there is only pain in it, it hurts a lot, I can't bear this pain in my toes and can't fold due to swelling.
