Black patches on hand and leg

As you can see in the image ...this black patches since more than year.. I have applied  few cream over it bt not shown very much effective... Recently I had done my blood sugar which is absolutely normal... Can u plz suggest me with this?

The picture are not so clear....pigmentation in the legs can be treated with de pigmentation  cream... Where as the one in the arm ....looks like a Tinea infection.... So its better to discuss in person
