Black itchy lesions on hands,legs n back

I have itchy lesions on my hands n legs since childhood. They become light on some areas n then appers on other parts.Currently the intensity is maximum on legs.The attached arm pic has a mild version.Could you please have a look n advise. Thanks in advance.
