3months old boy baby,he is crying for a week while feeding and when I keep him in shoulder he calm down. I consulted a doctor he said he is having an ear pain that's y he is refusing to feed

It could be associated with any viral illness. but if this symptoms persists, than get an ECHO done to rule out any heart related issues. Baby with heart disease might have difficulty in feeding, sweating during feeding , excessive cry and will Calm down if put to shoulder with knees fleXed. Although chances of heart disease is rare. If baby settled down in next few days, great but if persists review with your child specialist and if needed get an ECHO done.
