Based on the disease condition, it's severity, the time (chiefly the seasons), the individual body constitution determines the effect of medicine. From my experience I have observed that inn case of skin diseases, chronic rheumatological conditions, etc. the symptoms might increase initially. For initial 2-3 days, the symptoms may worsen. It is observed that the symptoms increase because already the cause of disease existing comes out. Then the root cause gets subsided. In younger individuals and initial stage of disease  cure is fast In older individuals and chronic diseases it takes time to cure. But the relief obtained by the cure is deeper. For purification therapies ( Panchakarma), Snehana, swedana as well the initial increase in symptoms mainly in aamata is observed. Later better cure is achieved.
