Anal fissures keep coming back

I keep getting anal fissure one in 3 months . It heals with sitz bath and ointment . This has been happening for nearly 8-10 years . Seeing blood makes me very anxious . I am postmenopausal on anti depressant drugs . Had sigmoscopy few years back which confirmed anal fissure with sentinel pile . How yo get a oermanent cure for this problem
