Allergic rashes/hives on skin

I have allergic skin condition since last 2.5 years. The rashes/hives occur at any time of the day at any part of the body and subside itself after sometime. Sometimes, bumps also get formed (even on scalp) and go away itself. Apart from this, I have persistent irritation and itching on my scrotum for around 2 years which is really disturbing me.
