How openly you get this pain  .. abnormality in the testis cannot give you pain or disturbance in your hand sometimes pain is there when you are too much into sexual thoughts without discharge temporarily you can use tight underwears or  supporters to subside the pain but better will be if you go for colour Doppler study of the scrotum and send the report or consult your family doctor or urologist
