A longtime hard lump behind ear

Hello. I have a girlfriend who has a lump behind her right ear only. The lump is hard and fixed in place, hurts a bit when pressed or sometimes just hurts a bit without pressing. She's had it for nearly 3 years till now but she hasn't gone to doctor yet and doesn't want to because she's highly afraid of sickness, that she even doesn't want to talk about it. I hope you can help us. Thanks a lot.
