White Spots on penis foreskin

I've rashes/vertical scales on penis head & white dots/spots all over penis foreskin. I've had consulted a physician 3-4 times earlier,he said its a penial skin infection as I did not have sex or any physicial activity or as such. Though medication helped somewhat but still spots & rashes are visible. It has been 4yrs since last consulted a doctor.
