During covid vaccination I noticed same used needle is used to extract vaccine but while injecting new needle is used, should I be concerned about this.

You should not be concerned about it as a vaccine vial contains more than one dose. So to extract dose from one vial they may be using one needle which is stuck to vial. For injecting it to you they will use new single use needle

I'm in-charge of Vaccination duty and I've been overlooking vaccination since about Feb. The syringes being supplied for vaccination is AD syringe i.e Auto Disable syringe so that it can be used once only. No where I've seen a practice like the one u r describing.
