If it is an isolated lymph node tb then there is no chance for transmission . However some patients may have the disease in lungs simultaneously which might be communicable to others . An X-ray chest screening is the primary modality to rule that out and if you have cough with expectoration then you can go for an Afb smear microscopy to rule out lung tb .

Extrapulmonary tuberculosis has low bacillary load and does not transmit through cough. However lung tuberculosis has to be ruled out first.
