Tiny cuts on penis

I am having tiny cuts at the foreskin of my penis. It hurts when I pull it back. I have used Cosvate GM cream for some time and they disappeared. But they keep coming back. My foreskin is tight also. But I am able to retract it back and forth when it is not erect. This could also be a problem why the cuts keep reopening. Please suggests some cream/medicine for the problem.
