Severe tinnitus with a sensation of ear blockage/pressure requires ENT evaluation to identify the underlying cause. Ear examination and hearing assessment PTA and tympanometry) are advised. Her post-meal blood glucose of 200–250 mg/dL is elevated and should be evaluated/managed by her physician. Further treatment will depend on clinical and audiological findings

Good afternoon Sir, I think she could have not just Ear but Nose too....There's a connecting tube between them and when it gets blocked one can feel blocking sensation in the ear as if pressure has built-up.

Please get done pure tone audiometry and impedance audiometry. It could be related to eustachian tube dysfunction. For High sugar consult physician once.

It may be due to nerve weakness, Vitamin deficiency will cause tinnitus problem Ear blocking condition may be due to, eustichian tube obstruction,  wax occumilation in the ear.

Further evaluation needed. Medication recommended.
