Although your echo and TMT are normal/negative but your complaints are very typical of heart pain (angina). Also TMT is not 100% accurate test to diagnose heart vessel blockages. It can have false negative results i.e. a negative result despite having heart vessel blockages. Although the most definitive test is an invasive conventional angiography but I think you should at least get a CT angiography of heart vessels done (CT coronary angiography). But before this you should meet a qualified cardiologist (DM or DNB cardiology) and get yourself properly examined and also discuss about this option of CT angiography. Sometimes these complaints can also happen due to spasms of foodpipe, so discuss this issue with your gastroenterologist as well. Hope this helps.
