Actually she is hypothyroid So u are giving her 100mcg externally.. It's ok for the dose when thyroid profile is in control But u need to check every 3months and adjust dose accordingly If the swelling or gland increases and associated with pain, difficulty in breating , difficulty in eating, loss of sweating, palpitations, chest discomfort, u need to consult Surgeon so as to know type of tissue in gland

Dear all History is not clear I like to know what medications she has taken for thyrotoxicosis. Let us discuss this aspect Treatment for Thyrotoxicosis ( Symptomatic Hyperthyroidism )  is entirely different from the Hypothyroidism What medications  your patient took for Thyrotoxicosis ??? We used to prescribe Anti Thyroid Drugs like  Carbimazile, Methimazole etc. for Thyrotoxicosis. But, T.Eltroxin for Hypothyroidism. Two possibilities are there 1st one is, Autoimmune Thyrotoxicosis like Graves disease, after some period of time  automatically  goes for Hypothyroidism 2nd one is, prolonged use of any anti thyroid tablets   without proper follow up can go for Hypothyroidism, which is called as "IATROGENIC Hypothyroidism, when the  doctors failed give advice for proper follow up. At the same time when the patient's side missed a follow up  called as  " Factitious Hypothyroidism" What is treatment concept in Thyroid diseases??? In thyroid disease ,whether it may be a hypothyroidism or hyperthyroidism Minimum 2 weeks required for the " Clinical/ Symptomatic improvement" ( i.e. patient can feel better from disease after 2 weeks). Then minimum 6 weeks required for  the significant " Biochemical Improvement ' ( i.e. to get significant lab report minimum 6 weeks required) Tis dictum to repeat TFT every 6th weekly with the adjustment of medications till Euthyroid  status ( i.e. till Thyroid Function Tests appears normal). Once Euthyroid is achieved continue the " SAME DOSE OF MEDICATIONS"  for next 3 months. And repeat TFT every 3 months once and alter the dose accordingly. "CERTAIN PATIENTS WITHHOLD/ STOP  MEDICINES AFTER  NORMAL LEVEL IS ACHIEVED" Which should not be done. Maintain the same dose till the next TFT value. Note :- *Hypothyroidism needs life long treatment *Thyrotoxicosis requires a maximum of 1.5 to 2 years of  treatment. After that medicines should be stopped When Thyrotoxicosis recurrence occurs after a period of 2 years treatment, surgery is preferred
