Throbbing pain in left hand and back

Having this throbbing pain in left hand side and back from time to time. Consulted many doctors but of no use.Swelling on left ankle too.Its in control for few days and then it's unbearable for few days.Have stopped feeling ok altogether.There is always a tightness in my chest.Because of this unable to live my daily life normally.Plz help. Regards
