The patient and doctor has to refer to the case and reference range that is applicable in the particular clinical scenario .
It’s a general reference range and not particularly for one particular individual
The lab and technician will not be aware of a particular individuals clinical status hence general ranges are mentioned
Pregnancy with thyroid is a special situation
So also thyroid in normal individuals is different
Hence everything is included
