Treatment of epilepsy depends upon a complete history and investigations like MRI brain ( epilepsy protocol) and EEG. Based on this a classification of focal versus generalsied epilepsy is made. Then one medicine is started and if seizures not controlled then the dose of medicine is increased. Another medicine is added if not controlled on maximum dose of one medicine. If patient is not controlled in 2 medicines with adequte dose then epilepsy surgery is to be planned for which patient needs to be admitted for VEEG , ICTAL SPECT to localise the focus of seizures and then that focus can be resected. If generalised seizure is found on evaluation and not controlled in medicines then DEEP BRAIN STIMULATION surgery is done as a palliative surgery.
