I have been suffering from Sternum and spine TB SINCE Feb 2017, now doctor advising me for Category IV treatment.

If you were taking regular and adequate treatment for TB from feb 2017, it must have healed by now. If it has not healed yet possibilities of drug resistant Tb are there. Please visit hospital to get a tissue sample for diagnosing MDR or XDR tuberculoses

If the tuberculosis has not responded despite your taking prolonged ATT REGULARLY. then category 4 is indicated. But I would suggest you add to that biopsy. Culture and resistance testing.
