I feel a bit of itchiness in the rectal area I have been diagnosed with a fissure A chronic one On the outside of my skin in anus The sensation does feel there Is it because of the fissure My dr has asked me to leave it as it is

Itchiness is rare in fissures. However it might be worms ,fungal infections other causes which can cause it. Consult  further.

If you have no problem with constipation now and no pain, then there is no indication for surgery. Try to use some sweat free powder for local application Laxative support might be useful Plenty of fibre diet and oral fluids
