Shoulder Pain Left side

I am having this problem since last 2 years, I am a software professional, have to work for about 10-12 hours a day. If the chair on which I am sitting is not properly setup, I feel pain my left shoulder(back side, a little below the shoulder) and hand, it feels to me that my left hand has became heavy.

Physiotherapy treatment and avoid computer work. Posture corrections must. Avoid strain till relief
