Sensitive acne prone skin

My skin is very sensitive and it's prone to acne breakouts easily. I don't use chemicals on face. Even then I keep getting acne. It's very mild acne only but it makes me look older than my age. I'd like to know what to do to cure my acne and small open pores problem. All I need is a clear skin. Kindly help.
