Pregnancy Related - no heartbeat in 8wks

Today, my wife had a eight week scan (LmP). but there was no heart beat. We already had one miscarriage in 2019. Unsuccessful IUI, and IVF in 2020. Also had fibroid removal surgery in December 2020. She conceived naturally this time but now no heartbeat. Is there any hope or its miscarriage. Following are the report findings: Clinical details: G-2 A- 1 Conception:Spontaneous Report says the following: also attached Gestational Sac seen. Sac margins appeared regular Gestational sac measured 30 X 13.8 X 15.4 mm.(Mean = 19.73) Large yolk sac seen Fetal pole seen Cardiac activity absent. Biometryy CRL-7.8 mm Impression POG BY (LMP) : 8 WEEKS 2 DAYS. SINGLE INTRÁUTERINE NONVIABLE GESTATION 6 WEEKS 5 DAYS MULTIPLE UTERINE FIBROIDS AS DESCRIBED. ------------ Kindly let me know. Thanks.

CRL of more than 7 with no cardiac activity is definitely a failed pregnancy/miscarriage . I would suggest you to consult an obgyn asap. You may need medications for evacuation of uterus. Also if you are planning for pregnancy again , first it is advisable to resolve the fibroids with medications (as they are too small) or if required removal of all fibroids first and then plan for pregnancy. Also tendency of fibroids reoccurrence is more specific in high aged women.
