I m 4 week pregnant. Should i need to go for a walk. And what foods i need to avoid. Also should i go for thyroid test

Yes thyroid test is important, so are all the ANC blood tests which is suggested on first visit. You can go for walk but don't lift heavy objects, no ling distance traveling for 3 months alteast. Take small frequent meals, avoid outside food
