Penis Discoloration & Urinary Hesitanc

I'm having discoloration of penis (black) under foreskin and penis develops smegma if i don't wash it. I also have pelvic floor dyssnergia, epididymitis, urinary hesitancy and frequent urination. What's wrong with my penis? Can urine problem be related to it or pelvic dysfunction?
