From your history, you have a deviated nasal septum (DNS) towards the right side with a septal spur, along with nasal obstruction and right-sided ear symptoms. The ear complaints include tinnitus, roaring sensation on posture change or forceful exhalation, and a feeling of heaviness, for which one ENT has diagnosed otitis media with effusion (OME), while others have considered sensorineural hearing involvement (nerve damage).
Both possibilities can coexist or mimic each other,
DNS with septal spur can compromise Eustachian tube function, leading to ventilation issues of the middle ear and OME, which explains ear fullness and roaring with pressure changes. On the other hand, tinnitus and heaviness may also be secondary to cochlear/retrocochlear pathology (sensorineural hearing loss), hence the concern about nerve involvement.
