Piles is a common term used for any complaint in the anus( part from where motion comes out). There are several diseases under that umbrella term piles. Medically we call only hemorrhoids as piles. Not all hemorrhoids need surgery. Only the severe grades require operation. Only examination by a surgeon and symptoms can assess the need for surgery
