Please suggest the next level of treatment or tests.Also pls clarify how this occured.please see the attached ultrasound report

Most common cause of this sort of picture is stones. If you have Pain in the back region and then it's the cause. Ideally you need a plain CT scan of KUB region to assess further. Based on a report, have to decide further.
