Lateral facelet problem

I have developed microchondrial softening and surface irregularity in lateral facelet of patella. Is there any chance it's lateral shift or tilt? MRI report shows patella femoral alignment is well maintained. what's the problem then?

Start static quadriceps exercises it will prevent patellar instability. Meanwhile visit a sports therapist for knee assessment.
