Jock itch problem

Hi doctor, I am suffering from jock itch for past years....what will be best medicine to take or any cream has to be applied?

SX – itch, burn, erythema, scales, hair loss (alopecia). PE – Skin. Hair/Scalp. Nails.

DX – Usually by Inspection only. Rarely: KOH microscopy, Wood Lamp exam, Culture or Biopsy.

TX – • Meds: griseofulvin 500-1000mg QD, itraconazole 100mg QD, terbinafine 250mg QD, tolnaftate topical, Fluconazole 200mg day 1, then 100mg QD. Antifungal choices include terbinafine, miconazole, clotrimazole, ketoconazole, econazole, naftifine, and (uncommonly) ciclopirox applied bid for 10 to 14 days. Itraconazole 200 mg po once/day or terbinafine 250 mg po once/day for 3 to 6 wk may be needed in patients who have refractory, inflammatory, or widespread infections.
