Itching and bad smell on the top part

Hello I am having some issue since last one year. I feel like bacterial infection. So I get sperm on the top part of the panish once I done masterbate. I have skin on the top part and it creates itching inside the skin of the top part also creates bad smell. So I do clean it regularly but it is happening since last one year before that I wasn't having any of those issue. Thank you.
