Continue Duphalac as prescribed for your fissure, maintain good hydration, gradually increase dietary fibre, and avoid unnecessary antibiotics or additional probiotics/antimicrobials on your own. Since symptoms have persisted for a month, if they do not improve despite constipation being controlled, I would recommend a clinical review rather than assuming it is SIBO/IMO.
