If the patient is on ARV and his viral load is undetectable on 2 occasions in last 6 months , there is no risk of transmission.
When we do not know the status of a person or about his Viral load , in such situations
HIV can be transmitted if an open wound is handled with bare hands where the skin is not intact or the skin has a open wound.
Next Steps
The patient needs to check his viral load to know if the ART medication is working or not. Which means if the patient has not developed resistance to the medicines.

Always use Gloves and treat all wounds as infectious.
Contact with blood or body fluids not only transmits
HIV but also Hepatitis B and Hepatitis C, which causes
liver damage if not tested and treated.
