Initial hard stool causing pain

For the past 1 year my initial stools are getting very hard and causes a lot of pain. I have tried all fibrous foods and laxatives but nothing works. There is even a skin tag that has formed now and swells up when passing hard stool. This also causes pain. Has my walls become too weak?
