I feel uncomfortable in social situation

I think I've had this problem my whole life, but have never decided to address it properly. I hate social situations as I always think too deeply into them. For example, I am always worried about what people think of me, if i'll embarass myself etc. Participating in a group situations makes my very anxious, especially with strangers. I hate one-on-one conversations because i feel under pressure to keep he conversation going. Due to this, I have felt depressed and have self harmed. Please help me
