How much extent of Skin expected to be pulled back by age of 2 years? Also is anything need to be done to maintain better hygiene considering this condition?

Poor genital hygiene may result in chronic infection causing adhesions between the inner foreskin and underlying glans penis as well as fibrosis resulting inability to retract the foreskin, called phimosis.
Next Steps
Putting a steroid cream on the foreskin up to 3 times a day for 1 month. This is to loosen the skin
Health Tips
But requires direct physical examination by a pediatrician to ascess its severity before complications occurs.

Yes. Its phimosis and ballooning should not happen normally. It may require circumcision in some cases. U need to meet nearby  pediatric surgeon, or if not possible then urologist.
