Perforations in both ears is invariably due to nose and sinus infections progressing backwards via throat and eustachian tubes onto middle ear. That needs to be confirmed by a non-contrast CT scan of your paranasal sinuses(including both mastoids) in direct coronal view, 1mm cuts. Once the nasal disease is treated, ears will stop discharging and after 3 months one ear can be operated upon and ear drum be grafted. Subsequently after another 3 months, second ear can also be taken up for surgery.

She seems to be suffering from exacerbation of chronic middle ear infection. Right now she requires antibiotic tablets and drops with pain killers for a week to reduce infections.
