High sugar inspire of taking insulin 4 t

I m taking lispro insulin 24 units 3 times and lantus 36 unit at bed time. Inspire my sugar levels are fasting 140-160 and p.p. 220. Age is 61 and wt. 88 kg. In addition jarrdiance 10 mg once and metformin 1 gm twice. Pls. Guide how I can improve.
