Age at onset, frequency, duration, location, quality, severity (0–10), relation to sleep, meals, stress, alcohol, specific foods, exertion, screen time
Associated symptoms: nausea/vomiting, photophobia, phonophobia, osmophobia, allodynia, aura symptoms
Disability: Impact on work/study, missed days, need to lie down in dark room
Medication use: Acute meds used (NSAIDs, triptans, combination analgesics, opioids, caffeine)
Past history: Head trauma, infections, seizures, psychiatric comorbidities, sleep disorders
Family history of migraine or other headache disorders
Lifestyle & triggers: Sleep pattern, caffeine, alcohol, nicotine, hydration, exercise, stress, screen time, posture/neck strain

Keep hydrated, reduce screen time, get 7-8 hrs of sleep in the meanwhile.
